Shepley recounts the reorganization of the Washington University School of Medicine and its affiliated hospitals into WUMSAH (Washington University Medical School and Affiliated Hospitals). He discusses the conflict between the School of Medicine and the board of Barnes Hospital, and the roles of the individuals involved in the formation of WUMSAH, including Edgar M. Queeny, Edward W. Dempsey, James S. McDonnell, and William H. Danforth.

Barnes Hospital was founded on an endowment established by Robert A. Barnes, a St. Louis wholesale grocer, who died in 1892. The original buildings were constructed between 1912 and 1914. In 1911, a contract was made between the hospital trustees and Washington University School of Medicine affiliating the two institutions, whereby the faculty of the school serves as the medical staff of the hospital.
